Student Eligibility*
--at time of application--
- Under age 30
- Currently enrolled in a state-accredited college or university as an undergraduate student taking 9 or more credits or a full-time graduate student. We will also accept the schools definition of a full-time student.
- Must remain a full-time student for at least 31 days following the date of application.
- May not have been previously declined for insurance due to preexisting health issues.
- May not have taken prescription medication for any medical condition (excluding birth control medication) in the past 15 days.
- May not have seen by a member of the medical profession for a medical condition in the past 15 days.
- May not have been confined to a hospital in the last 15 days.
- May not have been scheduled or recommended to have diagnostic testing, treatment, or surgery, that has not yet been completed in the past 12 months.

May not have received any tests, medical
treatment, surgical treatment, consulted a physician, or
taken medication in the past 5 years for the following conditions**:
- Heart disorder including heart attack or chest pain
- Stroke
- Diabetes
- Stomach or duodenal ulcer, ulcer symptoms, colitis, or Crohn's disease
- Uncorrected gall bladder disease or gallstones
- Kidney disease
- Cancer, tumor, or internal cyst
- Alcoholism or alcohol abuse
- Chemical dependency or drug abuse
- Our primary Student Health Insurance
product is available to students who are either legal
residents in 42 states in which we issue policies or are
attending school in one these 42 states.

In limited cases where student coverage is not available our parent company, Zane Benefits, offers similar policy coverage from traditional major medical policies, such as Blue Cross.
*For Assurant Student Select Medical Insurance. Zane Benefits recommends Student Select for students because coverage is available from doctors and hospitals anywhere in the United States. Student Select is not available to students who are both from (legal resident) and attending school in ME, MA, MT, NH, NJ, NY, RI, VT.
